2026 Application

The Application will open January 2, 2026.

Please complete this application so we can determine your eligibility for receiving funds set aside by the Blaine Scholarship Foundation (BSF) and any other organization or individual who has authorized the BSF to administer the awards of their scholarship funds. Students must satisfy the criteria developed by BSF and the Citizens’ Scholarship Foundation of America, Inc. (CFSA). Complete your sections of this application at your earliest convenience, then speak with a teacher about writing you a letter of recommendation. In addition to a teacher, include an additional recommendation from at least one employer, a member of the clergy, a job supervisor, or any other community leader who is in a position to evaluate you. Please also print out the Transcript Information Form and have your senior class counselor fill in the required information. You must then scan both the Transcript information Form and an official copy of your transcript and submit them along with your application. Keep in mind, your work in the application cannot be saved for later and must all be completed in one sitting. You may want to open the application and take note of the essay topics and write them before beginning the remainder of the application.

If any questions are not applicable to your current situation, please attach an explanatory note referring to the question by section. If more space is required for information on any items, you may attach additional information, indicating the appropriate sections.

Application Completion Criteria

This application will only be accepted by applying online at the Blaine Scholarship Foundation website.

Each scholarship application will be time-stamped and only accepted if the application falls within the application deadline. If you are unable to access these resources, please contact the Senior Class Counselor. You are responsible for seeing that all supporting documents are submitted. BSF reserves the right to process only applications found to be completed on or before the application timestamp deadline.

Remember, this application becomes valid only when the following have been submitted:

  • Completed Application
  • Essay
  • 2 letters of Recommendation
  • Transcript
  • Transcript Information Form

Total 100 Points
